Case 43: Direct composite veneers in esthetic management for Dental Fluorosis

Dental fluorosis is a situation when fluoride intake via water intake during childhood is beyond 2ppm especially during the phases of teeth development. Here is a case where the entire dentition is affected by dental fluorosis with severe brown stains on certain teeth.

The treatment option given to enhance the smile was indirect laminate veneers. But, the patient did not have much time in town and opted to go in for a transitional solution of direct composite veneers. Although it can hold good for quiet few years, a definitive solution in my opinion would be with laminate veneers.

Situations have a Dean's index of 1 can be treated with Resin infiltration technique as well. But this situation was a Dean's index 4 to 6 with some teeth showing evidence of pitting with stains and some teeth without staining. 

Since the patient wanted a quickfix just to mask his badly stained upper front teeth alone, I did a direct composite veneering for maxillary canine to canine in the esthetic zone. Filtek Z350 dentin and enamel were used and a shade of A2 was obtained. Further finishing and polishing was carried out using Shofu's Super snap mini and subsequent use of stoddard brushes and diamond coated spirals form ivoclar to get the desired finish.
