Case 18: Two Implant supported manibular overdenture

Sometimes, we come across patients who never accept a removable dentures and request for some form of fixed or semi-fixed( attachment supported) prosthesis. In this case the patient had adequate bone for a complete implant supported fixed rehabilitation but, economical constraints prevented him from getting it done. Instead he opted for two implant supported overdenture.

After CBCT analysis, I decided to go ahead with the fabrication of complete dentures first, followed by placement of two Standard diameter platform Biohorizon Implants with OD Secure attachments immediately loaded and picked up on to the denture chair side. OD secure attachments are versatile and I personally love their different emergence heights and their sizes.


The entire procedure was done under local anesthesia with a minimal flap exposure only to expose the site where the implants were planned. After the osteotomy completion. paralellism was checked with paralleling pins. Excellent primary stability was achieved and the OD secure abutments were torqued to 30Ncm and the denture was prepared for chair side pick up of the attachment housing.



 OD secure attachment kit normally has 4 different sleeves each of different retentive capacitites. Here since it was an immediate loaded over denture, I decide to choose the pink sleeve which has minimum retentive capacity and safe enough for immediate loading.

After placing the sleeve inside the housing, the complex is placed over the abutment just after placement of the protective ring as shown in the image. This prevents any excess resin to trap under the abutment and protects the site.

Chair side pick up acrylic resin which was auto-polymerising resin was used to pick up the retentive complex over the OD secure attachment. After the acrylic was set, the denture is removed and excess resin is trimmed off and occlusion assessed.
