Case 15: Smile Designing - An important tool in esthetic rehabilitation

Sometimes, Prosthodontic priniciples are broken and rehabilitation are done just to satisfy the need for replacing posteriors with fixed teeth despite the lack of posterior retainer abutments. This case was one such example, where the previous clinician gave a 10 unit fpd with distal cantilevering for pontics from 1.4 to 1.6. For replacing such a long span, DeVan's dictum was broken and unnecessary retainer support was taken from 1.3 to 2.4.



Not only the principles were compromised but also the esthetics and smile priniciples were hampered. After discussing with the patient, We decided to remove the existing unesthetic bridge and go ahead with smile designing and individual emax crowns for the prepared reatiners. Following a CBCT analysis, two implants were planned for site 1.4 and 1.6 followed by a 3 unit implant prosthesis.

The initial phase involved, the restoration of smile curve and esthetics, followed by implant placement at sites 1.4 and 1.6







As an interim measure until the implants are to be loaded, a removable prosthesis lined with soft- line was given.

The case is yet to be completed and the patient will be reviewed after 6 months for implant prosthesis.
